WebDischarge to Assess (D2A) is a model of hospital discharge which focusses on improving capacity for assessment and re-enablement1 to reduce delayed transfers of care (DTOCs). In Ipswich and East Suffolk, a D2A model has been implemented where patients are allocated to one of four pathways: • Pathway 0 (zero): the individual is Webcomplex pathway.
